for access to the personal data according to Art. 15 of the GDPR 

  • The form is designated to exercise the right of access to personal data recorded in SOLUS registers.

  • The form is designated for individuals and entrepreneurs.

  • PIf you are not satisfied with the electronic order form, you can apply by mail using this form.

    •  

  • We process an application meeting all the requirements without undue delay, but no later than within one month.

    •  

SOLUS has the right, in the case of a repeated or excessive application, in accordance with Article 12 (5) of Regulation (EU) 2016/679 of ohe European Parliament and of the Councilof 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (hereinafter referred to as “GDPR”) impose a reasonable fee of CZK 220, taking into account the administrative costs of providing the required information.
